COVID19: Self-Check
To streamline daily health check processes and reduce the spread of COVID-19 within our schools and community,
Cloverdale Unified School District is launching ParentSquare. Staff may go to ParenSquare's website and complete their daily screening to obtain clearance to go on site.
How to Complete the Health Screening Form

Open the ParentSquare app or website and click the Submit Daily Health Screening button on the home screen.

Answer for yourself (if staff member) and/or for students of which you are a parent/guardian.

Once you submit, you will see a pop-up with your clearance status.

You also can access all submitted screening forms and confirmations.
